Where can I get an authentic NFL jersey custom made?

Not only the name and number. What I do is have four numbers collectively different shirt. Example: # 51 back # 97 # in 27 front on the left sleeve # 55 on the right sleeve with the word "injury" in the rear or possibly "Wrecking Crew" or "Hit Squad". Not sure yet. Wonder if possible. Some good ideas, but I would the guy with the real numbers, embroidered Reebok. NFL Shop tried before posting questions. Thanks though.

Any place that is decent t-shirt printing must be able to do … if you can find a shirt with no number or name.
